What is the MILD® Procedure?
The MILD® Procedure (Minimally Invasive Lumbar Decompression) is a safe, outpatient treatment designed to relieve lower back and leg pain caused by lumbar spinal stenosis. The procedure removes a small amount of thickened ligament tissue in the lower back that is narrowing the spinal canal and compressing nerves — without the need for open surgery.
MILD® is performed through a tiny incision (smaller than a baby aspirin) and is an excellent alternative for patients who are not ready for, or wish to avoid, more invasive spine surgery.

How is the MILD® Procedure Performed?
The procedure is performed in an outpatient surgical setting under local anesthesia with mild sedation.
- You will lie comfortably on your stomach.
- A tiny incision (about 5mm) is made in the lower back.
- Using a specialized portal and fluoroscopic (X-ray) guidance, your provider inserts small tools to remove excess ligament tissue that is pressing on the nerves.
- No stitches are needed — the incision is so small that it can be closed with a simple bandage.
- The entire procedure typically takes about one hour, and most patients return home the same day.
What Should I Expect After the Procedure?
- Mild soreness at the incision site is normal and usually resolves quickly.
- You should limit strenuous activities for a few days after the procedure.
- Most patients resume light activities within 24–48 hours.
- Walking is encouraged shortly after the procedure to promote healing.
- Many patients experience significant improvement in their ability to stand, walk, and engage in daily activities within a few weeks.
Are There Risks or Side Effects?
The MILD® Procedure is extremely safe and has a very low complication rate. Rare potential risks include:
- Mild soreness at the incision site
- Bleeding or infection (very rare)
- Incomplete relief requiring additional therapy
- Temporary back stiffness
