What is a Pudendal Nerve Block?

A pudendal nerve block is a minimally invasive procedure used to treat pelvic pain, genital pain, and pain in the lower buttocks or perineum. The pudendal nerve supplies sensation and motor control to the pelvic region, including the bladder, bowel, and sexual organs.

By injecting local anesthetic and sometimes steroid medication near the pudendal nerve, this block can interrupt pain signals and provide relief from chronic pelvic pain syndromes.

How is the Procedure Performed?

The procedure is performed in an outpatient setting.

  • You will lie on your stomach or side to allow access to the pelvic area.
  • The skin over the lower back or buttocks is cleaned and sterilized.
  • A local anesthetic is used to numb the area.
  • Using fluoroscopy (live X-ray), ultrasound, or CT guidance, a small needle is placed near the pudendal nerve.
  • A combination of local anesthetic and corticosteroid is injected.
  • The procedure usually takes about 30–45 minutes, and you will be monitored briefly afterward before heading home.

What Should I Expect After the Injection?

  • You may feel immediate numbness or relief in the pelvic area due to the local anesthetic.
  • The steroid medication may take 2–5 days to fully reduce inflammation and provide longer-lasting relief.
  • Some temporary numbness, tingling, or weakness in the pelvic muscles is normal and should resolve within a few hours.
  • Avoid strenuous activity for 24–48 hours after the procedure.
  • Most patients experience significant pain relief, which can last from weeks to months depending on the condition.

Are There Risks or Side Effects?

Pudendal nerve blocks are generally safe. Possible, but rare, risks include:

  • Temporary numbness or weakness in the pelvic or buttock area
  • Mild bleeding or bruising at the injection site
  • Infection (very rare)
  • Allergic reaction to medication (extremely rare)
  • Temporary bladder or bowel changes
