What is a Superior Hypogastric Plexus Block?

A superior hypogastric plexus block is a minimally invasive injection used to relieve chronic pelvic, abdominal, and lower back pain. The superior hypogastric plexus is a network of nerves located in front of the lower spine that carries pain signals from the pelvic organs, including the bladder, uterus, prostate, and rectum.

By injecting local anesthetic and sometimes a steroid around this nerve bundle, we can block pain transmission and help improve your quality of life.

Why is a Superior Hypogastric Plexus Block Recommended?

How is the Procedure Performed?

The procedure is performed under sterile conditions in an outpatient setting.

  • You will lie on your stomach for the injection.
  • The skin over the lower back is cleaned and sterilized.
  • A local anesthetic is used to numb the area.
  • Using fluoroscopy (live X-ray) guidance, two small needles are inserted through the lower back toward the superior hypogastric plexus.
  • Anesthetic medication (and possibly steroid medication) is injected around the nerve plexus.
  • The procedure typically takes about 30–45 minutes, and you will be monitored for a short time afterward before going home.

What Should I Expect After the Injection?

  • You may feel numbness, warmth, or a heavy sensation in the lower abdomen or pelvis immediately after the procedure.
  • Pain relief can occur within a few hours but may take several days if steroid medication is used.
  • Mild soreness at the injection site is common and should resolve quickly.
  • Most patients are able to return to light activities the next day but should avoid strenuous activities for 24–48 hours.
  • Many patients experience significant improvement in pain levels, which can last for weeks to months.

Are There Risks or Side Effects?

Superior hypogastric plexus blocks are considered safe when performed by trained specialists. Potential but rare risks include:

  • Temporary low blood pressure or lightheadedness
  • Temporary bowel or bladder dysfunction
  • Infection at the injection site (rare)
  • Bleeding or nerve irritation (very rare)
  • Allergic reaction to medication
