Step 1: Provider Evaluation
Your healthcare provider will assess your condition and determine if DME is medically necessary. They will provide a prescription or order for the equipment.

- If you have Medicare, Medicaid, or private insurance, prior authorization may be required.
- You may be responsible for a co-pay or deductible, depending on your plan.
- Utah Medicaid and most private insurers follow DME authorization policies.
