What is a Trochanteric or Iliopsoas Bursa Injection?

A trochanteric bursa injection or iliopsoas bursa injection is a minimally invasive procedure used to treat pain and inflammation around the hip area.

  • The trochanteric bursa is located over the outside of the hip near the greater trochanter (the bony point you can feel)
  • The iliopsoas bursa is located near the front of the hip joint, underneath the iliopsoas muscle

If these bursae become irritated or inflamed, they can cause persistent hip pain — often called bursitis. An injection of corticosteroid medication directly into the bursa can reduce inflammation, relieve pain, and improve mobility.

Why is This Injection Recommended?

You may benefit from a trochanteric or iliopsoas bursa injection if you have:

  • Trochanteric bursitis (outer hip pain)
  • Iliopsoas bursitis (deep groin or hip flexor pain)
  • Hip pain that hasn't improved with rest, physical therapy, or oral medications
  • Difficulty walking, sleeping on the affected side, or exercising

The goal is to decrease inflammation in the bursa, allowing better movement and reduced discomfort.

How is the Injection Performed?

The procedure is typically performed in an outpatient setting:

  • You will lie comfortably on an exam table, depending on which bursa is being treated
  • The skin over the hip will be cleaned and sterilized
  • A local anesthetic may be applied to numb the skin
  • Using ultrasound or fluoroscopic (X-ray) guidance, the doctor carefully places a small needle into the bursa
  • A combination of a steroid medication and sometimes a local anesthetic will be injected
  • The entire procedure usually takes about 15–30 minutes

What Should I Expect After the Injection?

  • You may experience immediate relief from the local anesthetic, but this may wear off in a few hours
  • Steroid medication typically starts working within 2–5 days
  • Mild soreness or bruising at the injection site is normal and can be managed with ice packs
  • Limit strenuous activity for 24–48 hours after the procedure
  • Many patients experience significant pain reduction and improved function for weeks to months following the injection

Are There Risks or Side Effects?

While generally very safe, potential risks include:

  • Temporary increase in pain (rare and typically resolves within a day or two)
  • Infection at the injection site (very rare)
  • Bleeding or bruising
  • Steroid-related side effects (temporary blood sugar increase, flushing, or headache)
